Agreed Terms of Use: I agree to supply the Ardsley Education Foundation (hereinafter "AEF") with a post-implementation one page evaluation 3-6 months after implementation. Requests for additional post-implementation evaluations may be asked for from time to time, including photos and student statements. By execution of this form, the undersigned acknowledges that the AEF is merely funding the project and has no liability regarding the program, implementation and equipment, and the undersigned on behalf of the Ardsley School District, releases AEF and all of its officers, directors, and members and agrees to hold the same harmless from any and all loss, catastrophe and/ or injury in connection with the implementation of this grant.

PAYMENT TERMS: I agree that once AEF approves the project, in order to get a check cut to the Ardsley School District, actual quotes from approved vendors must be submitted to your school representative. At CRS-Toni Tiso, at AMS-Trish Soto, at AHS-Samantha Luis. Please write "yes" if you agree. *
